The past medical history section of the Med+DBase patient record allows you to enter data directly in to the medical record, giving it meaning and coding.
Data should be entered into the medical history log when clinicians or admin staff need to add key data to the medical record. The log function should not be used for recording consultation data. This should be done via the appointment.
To log data into the medical record, simply type into the log text box at the top or the page. Med+DBase will also auto-predict words.
Hit the Log button, located to the right of the text box to add the details to the medical record. You will be prompted with a drop down list of log type. Select on and hit Submit
Your details will be saved to the patient record under the data type selected
